Purpose of this Form
This form is used by a Scouts ACT staff or other authorised persons to collect and verify relevant details from individuals who intend to use Scouts ACT motor vehicles but prefer not to upload a copy of their driver's licence for security reasons.
For the purposes of this form, an authorised person is a Scouts ACT:

Procedure
An authorised person must physically view the individual's driver's licence (front and back) and record the relevant details below.
Record the following details exactly as they appear on the individual's driver's licence:
The authorised person who verifies the licence must confirm that they have sighted the driver's licence and recorded the details accurately.
A copy of this form will be emailed to the individual to attach to their Scouts ACT Motor Vehicle Use Agreement.
Do not photocopy, scan, photograph, or retain a digital or physical copy of the driver's licence.
If the individual continues to use Scouts ACT motor vehicles, this process must be repeated annually. A new form must be completed and the driver's licence must be sighted again to ensure the recorded details remain current.
Any discrepancies, concerns, or issues identified during the verification process must be escalated to the Executive Officer.
This form is intended solely as evidence that the driver's licence has been sighted and verified. It does not authorise the individual to operate a Scouts ACT motor vehicle. Authorisation is subject to the completion and approval of the Scouts ACT Motor Vehicle Use Agreement and compliance with the Scouts ACT Motor Vehicle Use Policy.
