This form is for new members (including members aged 16 providing their first WWVP card) and existing members who are required to submit an updated WWVP card.
Please upload clear images of both the front and back of your current WWVP card.
Requirements:
The card must be current and not expired
Both sides of the card must be uploaded
Your full name, registration number, and expiry date must be clearly visible
Images must be clear, in focus, and not cropped
Temporary, digital, or receipt‑based evidence cannot be accepted.
Under ACT law, you are required to carry your WWVP card with you whenever you are engaging in activities involving children and young people. Please ensure your card is kept accessible at all times.
Important compliance notice:
For new members, incomplete or unclear documentation may delay the processing of your membership or appointment.
For existing members, failure to hold and provide evidence of a current WWVP card will result in termination of membership, as members must hold a valid WWVP registration at all times.
